Application to Fundraise or Collect Items for Charity

The purpose of this form is to gather information so administrative staff can decide whether to approve a fundraising or collection event by a church committee or task force.

When you submit this form, your entries will be sent to Amanda Waye, Director of Administration, and the Lead Minister.

The name of the primary contact person for the proposed event. We will contact you with any and all questions about the event, or about the goals of the fundraiser/collection.

This is provided so that staff can make sure the church doesn't have too many fundraisers happening at the same time, NOT to schedule your event on the calendar. Once your fundraising application is approved, you will need to fill out a scheduling form.
